Document patient care in a manner that reduces the likelihood that medical errors, including misdiagnoses, will occur. 6. Discuss five most common sources of medical error and missed diagnoses, and ways to prevent them. 7. Know how to investigate adverse outcomes to determine the root cause, and take appropriate action to prevent recurrence.
